Please complete and sign the enclosed Student Records Request form so that we can forward it to your child's former school to obtain official records. Prior to registration, you must check on the arrival of the official records by calling the Pupil Personnel Services office at 815-462-2222. Please make a registration appointment as soon as possible after we receive the records. We expect parents/guardians and students to be present at registration. The first full day of attendance shall generally be the next school day after the registration appointment. (For incoming Freshmen living in district, not registered, appointments will be taken after April 1. For summer enrollees, appointments will be taken after June 15, in the order in which the calls are received.)
The following items are mandatory for registration:
1. School records – transcripts/past and current report cards, current schedule and test scores.
2. Certified Birth Certificate - copies are not accepted.
3. Illinois State Board of Education Student Transfer Form (applies only to students who transfer from an Illinois Public School). This form can be obtained from the previous school attended.
4. Residency Verification Form – form can be printed from the Lincoln-Way Central
Website (www.lw210.org/central/pps) or picked up in the PPS office.
5. Immunization Records - incoming freshman and out-of-state transfer students without ninth grade physicals will be required to have a health exam completed by the first day of student attendance.
A special note: If natural parents are divorced or separated, residential custody papers must accompany the child before registering. If not living with a natural parent, proof of court-appointed legal guardianship must be presented with a completed and approved Affidavit of Residency form. You must obtain approval from the principal prior to the registration appointment.
Lincoln-Way Central High School does not accept nonresident students. Parents/guardians and students making fraudulent registration will be subject to removal from school immediately.
